The pharmacy at DIHFS has added a Hypertension (high blood pressure) Management Program to help patients reach their goal blood pressure of <130/80 mmHg. We will work with your DIHFS providers to individualize care for patients with uncontrolled high blood pressure.
Once enrolled, the patient will meet with a pharmacist to discuss blood pressure monitoring and how medications and lifestyle changes can help to control their blood pressure. As part of the program, an electronic blood pressure monitor and cuff will be provided free-of-charge for at-home monitoring and the patient will keep a record of the readings. By clinic appointment or by phone call, a pharmacist will record the blood pressure readings, adjust medications, and discuss lifestyle modifications that will help to get the patient’s blood pressure under control quickly and safely.
